Fungal (mold) sensitization is an underrecognized driver of severe asthma and allergic bronchopulmonary aspergillosis (ABPA). Alternaria alternata (Alt a 1) is the most clinically important outdoor mold allergen, strongly associated with severe and life-threatening asthma exacerbations. Aspergillus fumigatus allergens (Asp f 1–6) define the spectrum from simple IgE sensitization to ABPA, a complex allergic-immunologic lung disease requiring distinct management. Recombinant fungal allergens enable component-resolved diagnosis of mold allergy and differentiation of ABPA from simple Aspergillus sensitization.

Alt a 1 (Alternaria alternata) — The major Alternaria allergen, a unique dimeric protein with no known homologs. Alt a 1 sensitization is recognized by >90% of Alternaria-allergic patients and is a strong independent risk factor for severe asthma, near-fatal asthma episodes, and thunderstorm asthma outbreaks. Recombinant Alt a 1 is the key diagnostic component for Alternaria sensitization.

Asp f 1 / Asp f 2 / Asp f 3 / Asp f 6 (Aspergillus fumigatus) — Aspergillus allergens define the clinical spectrum from simple sensitization to ABPA. Asp f 1 (ribotoxin) and Asp f 3 (peroxisomal protein) indicate simple Aspergillus sensitization; Asp f 4 and Asp f 6 are secreted allergens associated with ABPA and indicate fungal colonization. This CRD pattern (Asp f 1/3 positive vs Asp f 4/6 positive) differentiates simple sensitization from ABPA, guiding antifungal and corticosteroid therapy decisions.

Cla h (Cladosporium herbarum) / Pen ch (Penicillium chrysogenum) — Additional outdoor (Cladosporium) and indoor (Penicillium) mold allergens. Cladosporium is the most abundant airborne mold worldwide but less clinically severe than Alternaria. Penicillium sensitization is associated with damp indoor environments and sick building syndrome.
