Sjögren's syndrome (SS) is a systemic autoimmune disease primarily affecting the exocrine glands, characterized by lymphocytic infiltration of salivary and lacrimal glands causing progressive sicca symptoms (dry mouth, dry eyes). Anti-SSA/Ro (Ro52/TRIM21 and Ro60/TROVE2) and anti-SSB/La autoantibodies are the serological hallmarks, with anti-Ro52 being the most prevalent specificity and carrying neonatal lupus risk in pregnant patients. Systemic manifestations include interstitial lung disease (ILD), renal tubular acidosis, peripheral neuropathy, and a 15–20 fold increased risk of B cell non-Hodgkin lymphoma. Elevated BLyS/BAFF drives pathogenic B cell survival, germinal center-like structures in salivary glands, and autoantibody production. A type I IFN signature is present in ~50% of patients. Treatment remains largely symptomatic; biologics under investigation include belimumab (anti-BLyS), ianalumab (anti-BAFFR), and rituximab (anti-CD20).

Ro52/TRIM21 & Ro60/TROVE2 (Anti-SSA Autoantigens) — Anti-SSA autoantibodies targeting Ro52 (TRIM21) and Ro60 (TROVE2) are present in 60–80% of primary SS patients and are included in the ACR/EULAR classification criteria. SSB/La (Anti-SSB Autoantigen) — Anti-SSB/La autoantibodies target the La RNA-binding protein involved in RNA polymerase III transcription termination and are present in 30–50% of SS patients, almost always co-occurring with anti-SSA. Anti-SSB positivity is associated with longer disease duration and glandular dysfunction. IFN-α / IFNAR — A type I IFN signature is present in approximately 50% of primary SS patients and correlates with higher disease activity, anti-SSA positivity, and systemic manifestations. The IFN-α/IFNAR pathway overlaps significantly with SLE pathogenesis.
