Hashimoto's thyroiditis (HT) is the most common autoimmune disease and the leading cause of hypothyroidism in iodine-sufficient regions, affecting up to 5% of the general population with a strong female predominance. The disease is characterized by progressive lymphocytic infiltration of the thyroid gland, follicular cell destruction, and fibrosis. Both cellular (cytotoxic T cell-mediated) and humoral (anti-TPO, anti-Tg autoantibodies) immune mechanisms drive thyroid destruction. Genetic susceptibility loci include HLA-DR3/DR5, CTLA-4, and PTPN22, with environmental triggers such as iodine excess, viral infections (HCV, SARS-CoV-2), and micronutrient imbalances (selenium deficiency) contributing to disease initiation.

TPO (Thyroid Peroxidase) — Anti-TPO autoantibodies are the most sensitive serological marker for Hashimoto's thyroiditis, present in >90% of patients. TPO catalyzes thyroid hormone synthesis and is the primary autoantigen driving complement-mediated thyrocyte destruction. Recombinant anti-TPO antibodies (multiple clones: RI-34, RI-33, RI-8) with fluorescent conjugates (FITC, PE, APC, PerCP) support flow cytometry, ELISA development, and epitope mapping.

Thyroglobulin (Tg) — Anti-thyroglobulin autoantibodies are present in 60–80% of HT patients and serve as a complementary diagnostic marker alongside anti-TPO. In differentiated thyroid cancer follow-up, anti-Tg antibodies can interfere with serum Tg measurement, making antibody characterization critical for assay development. TSHR (TSH Receptor) — While TSHR-stimulating antibodies define Graves' disease, TSHR-blocking antibodies are found in a subset of HT patients and contribute to hypothyroidism. Anti-TSHR recombinant antibodies (M22, K1-70, 3BD10, 1H7) with multiple conjugates support receptor binding assays, functional blocking studies, and differential diagnosis research between stimulating and blocking TSHR autoantibodies.
