p53 needs no introduction. Encoded by the TP53 gene, it is the most frequently mutated gene in human cancer — altered in roughly half of all solid tumors. As a transcription factor, wild-type p53 responds to DNA damage, oncogene activation, and other stress signals by inducing cell cycle arrest, DNA repair, or apoptosis. When p53 is mutated, it typically loses this tumor-suppressive function and, in many cases, acquires oncogenic gain-of-function properties that actively promote tumor progression.

Standard anti-p53 antibodies detect both wild-type and mutant p53 protein. In many tumors, this is fine — missense mutations in TP53 stabilize the protein, causing it to accumulate to high levels that are easily detected by IHC as strong nuclear staining. But this "pan-p53" detection cannot tell you which mutation is present, or distinguish wild-type p53 stabilized by MDM2 inhibitor treatment from mutant p53 stabilized by loss of degradation.
The R175H mutation is the single most common TP53 hotspot — it accounts for approximately 6% of all p53 mutations across cancer types and is especially frequent in ovarian, colorectal, and breast cancers. R175H disrupts the zinc-binding domain, causing a dramatic conformational change that abolishes DNA binding and exposes neoantigenic surfaces not found on wild-type p53.

p53 IHC is one of the most commonly ordered stains in surgical pathology research, but interpreting it requires understanding what you are actually seeing. The staining pattern reflects the mutation status of TP53 in the tissue:
| Strong diffuse nuclear | Suggests missense mutation (stabilized mutant protein accumulates). The most common IHC pattern in p53-mutant tumors. |
| Complete absence | Suggests nonsense or frameshift mutation (truncated protein not detected by antibody) or homozygous deletion. The "null" pattern. |
| Wild-type pattern | Scattered positive nuclei among negative cells — normal p53 turnover. Positive cells represent transient stabilization during cell cycle checkpoint activation. |
| Cytoplasmic staining | Rare but can indicate specific mutations that impair nuclear localization, or antibody artifact. Verify with a second clone. |
